HOUSEHOLD INFORMATION FORM
The Chapel of Saint John the Divine is collecting information for the church directory and for the Prayers of the People on Sundays Having the following information in our office records helps us to be in contact with you, and to be more connected as a church family. Please complete full household address information, and contact information for at least one individual at your address. The additional information is very helpful to us, too, so please provide us with as much of the information for additional individuals as you know, or is applicable to the person. This form is meant for individuals living at the same address. You may have children living at other addresses - if so, we'll keep your child's name in your records, but please submit a separate form for their address. This includes college students! You can also use this form to update your address, email address, phone number, or other contact and personal information. Just provide the new information in the appropriate places below, and we'll update our records promptly. Privacy Notes: 1. Data Sharing - Member names, addresses, phone numbers, and email addresses will be printed in the official Church Directory, unless a member has specifically requested that this information be kept private. All other information requested here is strictly for church internal records and operations. It will not be distributed, published, or shared in any way other than in-house, and as may be needed for pastoral/congregational care purposes.
